Trans Abstract

Background

Unilateral peripheral facial nerve palsy may have a detectable cause (secondary facial nerve palsy) or may be idiopathic (Bell’s palsy). Facial palsy is attributable to various causes ranging from mild infection to severe neurological disorders. We investigated the prevalence and types of serious neurological disorders in patients with unilateral facial palsy.

Methods

We reviewed the medical records of patients with unilateral facial nerve palsy and identified patients diagnosed with facial palsy secondary to serious or life-threatening causes. We investigated the clinical characteristics, as well as electrodiagnostic and imaging findings in these patients.

Results

Of 924 patients with facial palsy, 11 patients (1.2%) were diagnosed with the following serious neurological disorders: acoustic schwannoma in two patients, facial nerve schwannoma, glossopharyngeal schwannoma, meningioma, epidermoid cyst, parotid gland tumor, pontine infarct, skull base osteomyelitis, brain metastasis, and pachymeningitis.

Conclusions

Although unilateral facial palsy is rarely associated with serious neurological disorders, early detection of the etiopathogenetic contributors is important for prompt initiation of optimal management. Therefore, clinicians should be mindful of disorders that can mimic Bell’s palsy.

서론

급성 일측성 안면마비를 유발하는 가장 흔한 질환은 뚜렷한 원인이 없이 발생하는 벨마비이다[1,2]. 이차성 원인은 Ramsay-Hunt 증후군, 수술, 외상, 전신바이러스 감염, 국소적인 감염, 면역질환, 종양 등으로 다양하다. 수술이나 외상은 병력에서 확인이 가능하고, Ramsay-Hunt 증후군은 피부 병변이 동반되며, 감염의 경우는 발열 같은 전신 감염 증상이 동반되기 때문에 벨마비와는 구별이 된다. 하지만 종양이나 뇌경색 같은 중증 신경학적 질환이 안면마비로만 발현하는 경우는 의심하기가 어렵고, 진단이 늦어지면 심각한 신경학적 결손을 초래하거나 치료 시점을 놓칠 수 있다. 안면마비로 발현하는 중증질환에 대한 증례보고는 있지만 대규모 안면마비 환자를 대상으로 조사한 보고는 없었다[3,4].

10년 동안 벨마비 증상으로 내원한 환자를 대상으로 중증 신경계 질환에 의한 환자의 빈도를 알아보고, 벨마비 증상으로 발현할 수 있는 중증 신경계 질환의 종류 및 조기진단에 도움이 되는 위험징후가 있는지 알아보고자 연구를 시행하였다.

대상과 방법

1. 대상

본원 신경과의 안면마비 환자 등록대장을 토대로 2010년 11월 1일부터 2020년 10월 31일까지 벨마비 증상(급성 일측성 안면마비)으로 내원한 환자의 의무기록을 검토하였고, 중증 신경계 질환으로 인한 안면마비로 진단받은 환자를 대상으로 하였다. 중증 신경계 질환은 종양이나 뇌경색 같이 구조적인 이상이 있어서 진단을 위해서는 영상 검사가 필요하고, 적절한 치료를 하지 않으면 호전되지 않는 질환으로 정의하였다. 안면마비 외에 다른 신경학적 이상이 동반된 경우는 제외하였다. 뇌영상 검사에서 안면신경마비와 무관한 중증 신경계 질환이 관찰된 경우도 제외하였다. 연구는 본원 연구윤리심의위원회의 승인 하에 이루어졌다 (CR-21-044-L).

2. 방법

중증 신경계 질환에 의한 안면마비로 진단받은 11명을 대상으로 의무기록을 후향적으로 검토하여 임상 양상과 검사 결과를 기술하였다. 대상자의 나이, 성별, 동반질환, 안면마비의 중증도, 과거 안면마비 병력, 안면마비의 동반 증상, 전기진단 검사 결과, 뇌영상 검사 결과, 최종진단을 조사하였다. 안면마비의 중증도는 HB 척도(House-Brackmann grade)를 이용하여 평가하였다[5]. 안면마비의 동반 증상은 귀 주위 통증, 미각 변화, 청각과민 유무를 확인하였다. 전기진단 검사는 안면신경전도 검사와 순목반사 검사를 시행하였다. 안면신경전도 검사에서 복합근육활동전위의 크기와 순목반사 검사에서 R1, R2 파형의 유무와 잠복기 연장 여부를 토대로 판독하였다. 뇌영상 검사는 신경영상을 전공한 영상의학과 전문의가 판독하였다.

결과

10년 동안 비외상성 급성 일측성 말초성 안면마비로 내원한 환자는 총 1,071명이었다. 이 중에서 추가 검사나 치료를 받지 않고 한번 내원 후에 추적 중단된 147명을 제외한 924명 중에 영상 검사에서 중증 신경계 질환을 진단받은 환자는 총 11명이었다. 영상 검사는 924명 중에 833명(90%)에서 시행하였다. 뇌 자기공명영상 검사(magnetic resonance imaging, MRI)를 시행한 환자가 724명이었고, 뇌 컴퓨터단층촬영이 80명, 확산강조영상 검사 13명, 뇌 컴퓨터단층촬영과 확산강조영상 검사를 모두 시행한 환자가 16명이었다. 이 중에서 48명은 외부에서 검사한 영상자료를 가져왔고, 나머지는 본원에서 촬영하였다. MRI를 시행한 724명 중에 630명은 내이도 MRI를 시행하였고, 나머지는 뇌 MRI를 시행하였다.

안면마비의 임상 양상과 인구학적 특성은 Table 1에 정리하였다. 여자 환자가 6명이었고, 평균 나이는 60.5세(범위, 37-81)였다. 전기진단학적 검사는 9명에서 시행하였고, 9명 모두에서 이환된 쪽의 안면신경마비 소견이 관찰되었다. 영상 검사는 모든 환자에서 시행하였다. 11명 중에 8명은 증상 발생 후 4일 이내에 본원에 내원하였다. 이 중에서 7명은 바로 영상 검사를 시행하였고, 2명은 안면마비 정도가 점차 심해져서 18일, 90일째에 시행하였다. 나머지 3명은 외부병원에서 영상 검사 없이 치료하다가 증상이 지속되어 본원에 내원하였기 때문에, 2명은 증상 발생 2달 뒤, 한 명은 3달 뒤에 영상 검사를 시행하였다. 최종진단은 청각신경 집종이 2명으로 가장 많았고, 혀인두신경집종, 이하선종양, 두개저골수염, 뇌교경색, 수막종, 표피모양낭종, 전이성종양, 경수막염, 얼굴신경집종이 각각 1명이었다(Fig. 1). 혀인두신경집종은 이하선안쪽으로 커져서 이하선에서 안면신경을 압박하였고(Fig. 1E), 수막종과 표피모양낭종, 청각신경 집종은 소뇌교뇌각(cerebellopontine angle) 부위에서 안면신경을 압박하였다(Fig. 1C, F, I). 뇌교경색은 안면신경의 핵이 위치하는 뇌교의 등쪽에 국소적으로 관찰되었다(Fig. 1L). 10번 환자는 영상 검사에서 비인두암이 의심되는 소견이었으나 조직 검사를 통해 두개저골수염으로 최종진단이 되었다(Fig. 1K).

얼굴신경집종, 청각신경집종, 수막종 환자(1-3번 환자)는 감마나이프 수술을 포함한 수술적 치료를 위해 외부병원으로 의뢰되었고, 4명은 추적이 중단되었다. 특별한 치료 없이 2년 동안 추적 관찰한 5번 환자는 안면마비의 정도가 호전되지 않고 그대로 지속되었다. 본원에서 치료를 시행한 9, 10, 11번 환자는 치료 후에 안면마비가 완전히 회복되었다.

고찰

급성 안면마비는 대부분 특별한 원인이 없는 벨마비에 의해 발생한다. 이차성 원인으로는 전신바이러스 감염, 외상, 수술, 당뇨, 국소 감염 등이 있으며 드물게 뇌종양과 뇌경색 같은 중증 신경계 질환에 의해 발생 가능하다. 1,2 급성 안면신경마비 환자를 진료할 때 중증 신경계 질환의 빈도가 낮기 때문에 영상 검사를 모든 환자에서 시행하지는 않는다. 임상 양상이 비특이적이거나 일반적인 회복경과를 보이지 않는 경우에만 뇌 병변 등의 원인 감별을 위해 영상 검사를 시행하는 것이 보편적이다[6,7].

안면신경은 뇌교에 위치한 안면신경핵에서부터 시작한다. 안면신경의 운동핵은 뇌교 덮개의 배가쪽에 위치하고, 뇌교를 나오게 된 안면신경의 주행경로는 6개의 구간으로 나누어 설명할 수 있다[8]. 첫 번째 구간인 두개내 분절(intracranial segment)은 소뇌교뇌각에서 내이도 입구까지이다. 두 번째 이도 분절(Meatal segment)은 측두골의 내이도 구간이다. 세번째 미로 분절(Labyrinthine segment)은 내이도 출구에서 슬신경절까지 구간이다. 이후에 네 번째 고실 분절(tympanic segment)을 지나 다섯 번째 유양돌기 분절(mastoid segment)로 이어진다. 유돌 분절은 경유돌공(stylomastoid foramen)까지의 구간이다. 경유돌공을 나온 안면신경은 마지막으로 외측두엽 분절(Extratemporal segment)을 형성하며 이하선 내에서 분지한다. 따라서 뇌교에서 이하선까지의 모든 경로에서 안면신경이 침범되면 벨마비와 유사한 안면신경마비로 발현할 수 있다. 본 연구에서도 뇌교의 안면신경핵에서부터 이하선까지 다양한 부위에서 병변이 관찰되었다. 영상 검사를 판독할 때 안면신경마비를 초래할 수 있는 질환의 종류와 해부학적인 위치를 염두에 두는 것이 도움이 된다[9,10].

얼굴신경집종은 안면신경 6개의 분절 어디에서나 발생할 수 있으며, 여러 분절에 걸쳐서 발생하는 경우가 흔하다. 이 중에서도 가장 흔하게 발생하는 부위는 고실 분절과 유양돌기 분절이다[8]. 청각신경집종과 얼굴신경집종은 영상 검사 만으로는 구분이 어려울 수 있다. 1번 환자는 유양돌기분절(mastoid segment)까지 종양이 관찰되기 때문에 얼굴신경집종으로 진단하였고[11], 8번 환자는 낭성변화를 동반한 특징적인 소견을 보여 청각신경집종으로 진단하였다[12]. 3번 환자처럼 내이도에만 국한된 경우에는 두 질환을 구분하기 어려우나 청각신경집종의 빈도가 휠씬 높기 때문에 청각신경집종의 가능성이 높다고 판단하였다.

중증 질환에 의한 안면마비가 드물고 대규모 연구가 없었기 때문에 중증 신경계 질환을 시사하는 소견에 대해 알려진 것은 별로 없다. 우리 환자들의 임상적 특징을 살펴보면, 5명(3, 4, 7, 8, 10번 환자)은 처음에는 벨마비에 준해서 치료를 하였으나 안면마비가 호전되지 않거나 점차 심해지는 경과를 보였고 이것은 3개월 내에 뚜렷한 호전을 보이는 벨마비와는 맞지 않는 소견이다. 얼굴신경집종을 진단받은 1번 환자와 청각신경집종을 진단받은 3번 환자는 이전에도 같은 쪽에 안면마비가 발생했다가 회복한 병력이 있었다. 재발성 안면마비의 이차성 원인을 조사한 연구에 의하면, 동측에 재발한 경우에 얼굴신경집종, Ramsay-Hunt 증후군, 이하선 병변이 관찰되었고 양쪽에 번갈아 가며 재발한 경우에는 Melkersson-Rosenthal 증후군, 신경사르코이드증, 베게너육아종증이 원인질환으로 관찰되었다[13]. 전이성 뇌종양을 진단받은 6번 환자는 폐소세포암으로 항암 치료와 방사선 치료 중이었다. 두개저골수염을 진단받은 10번 환자는 조절되지 않는 만성 중이염을 앓고 있었다. 뇌교경색을 진단받은 11번 환자는 고혈압, 당뇨, 고지혈증이 동반되어 있었고, 이전에 뇌경색 병력이 있었다. 이하선 내부의 종양을 진단받은 4번과 7번 환자는 진단 후에 실시한 신체진찰에서 이하선이 약간 부어있고 덩어리가 만져졌다. 따라서 증상 호전이 보이지 않는 경우, 재발성일 경우, 특정 동반질환이 있는 경우는 초기에 영상 검사를 하는 것이 도움이 되겠다. 또한 신경학적 진찰과 함께 이하선에 대한 문진이나 촉진을 실시하는 것도 이하선 내에서 발생하는 안면신경마비의 원인을 조기에 찾는 데 도움이 된다.

본 연구는 안면신경마비 등록대장을 토대로 하였으나 자세한 자료는 후향적으로 진료기록을 검토하여 얻은 연구라는 제한점이 있다. 그리고 단일 3차 진료기관의 결과이기 때문에 일반화하기 어려운 점도 있다. 또한, 본원이 안면신경마비 환자에서 영상 검사를 시행하는 빈도가 높은 편이기는 하지만 모든 환자를 대상으로 한 것이 아니기 때문에 놓친 신경계 질환이 있을 수도 있다. 하지만 대규모 환자를 대상으로 벨마비 유사 증상으로 나타날 수 있는 중증 신경계 질환을 연구하였다는 의미가 있다. 알려진 대로 중증 신경계 질환은 드물었지만, 진단이 늦은 경우에 심각한 결과를 초래할 수 있기 때문에 안면마비로 발현할 수 있는 질환에 대해 염두에 두는 것이 필요하다.
