Hereditary spastic paraplegia (HSP) is a heterogeneous group of monogenic neurodegenerative disorders characterized by progressive spasticity of the lower limbs. The clinical features and imaging abnormalities vary greatly according to the affected genes. HSP is classified clinically as pure and complex forms, depending upon the presence or absence of additional neurological defects other than spastic lower limbs. Despite the recent advances in next-generation sequencing technology and its wide availability, a genetic diagnosis of HSP is still not made in more than half of all suspected cases of HSP. In this review, we summarized the various phenotypes of relatively common HSP in clinical practice according to the inheritance pattern, highlighting their clinical, radiological, and neurophysiological features. We further discussed the practical approach to patients with suspected HSP in the current era of next-generation sequencing.
유전강직하반신마비(hereditary spastic paraplegia)는 단일 유전자변이에 의한 신경퇴행성 질환으로 변이 유전자의 종류에 따라 다양한 임상 양상을 보일 수 있다. 전 세계적인 유병률은 10만 명당 2-10명 정도로 보고되고 있으나[
유전강직하반신마비 환자에서 뒤섬유기둥(dorsal column) 퇴행이 동반되는 경우가 흔하고 피질척수로 이외에도 추체외로, 말초신경 등 다른 신경계가 침범되기도 하나, 큰 범주에서 운동신경세포병의 한 스펙트럼으로 분류할 수 있다. 운동신경세포병은 위운동신경세포 침범 여부에 따라 위운동신경세포병과 아래운동신경세포병으로 구분할 수 있는데, 위운동신경세포병의 대표적인 형태에는 원발성측삭경화증(primary lateral sclerosis), 유전강직하반신마비 등이 있고, 아래운동신경세포병의 대표적인 형태로 척수근위축증(spinal muscular atrophy), 진행성근위축증(progressive muscular atrophy), 원위유전운동신경병(distal hereditary motor neuropathy) 등이 있다(
유전강직하반신마비는 피질척수로의 퇴행성 변화에 의한 진행성 하지 강직을 특징으로 하는 단일유전자변이로 인한 질환으로, 대부분의 환자는 서서히 진행하는 보행장애를 주소로 병원에 내원한다. 변이 유전자의 종류에 따라 발병시기가 다양하여 영아, 유아, 청소년기에 발병하기도 하고 30-40대 성인이 된 이후에 첫 증상이 나타나기도 한다. 그 외 뒤섬유기둥 침범으로 인한 진동 및 위치 감각 저하와 빈뇨, 절박뇨 등 소변 증상이 흔하게 동반된다. 상지의 강직은 없으나 심부건반사가 항진되어 있는 경우가 많고, 경우에 따라 아래턱반사가 항진되기도 한다. 임상 양상에 따라 진행성 하지 강직과 일부 소변 증상, 하지 감각저하만을 보이는 순수형(pure form)과 인지기능저하, 시신경위축, 추체외로 증상, 소뇌실조, 말초신경병 등을 동반한 복합형(complex form)으로 구분한다[
유전강직하반신마비의 확진을 위해서는 원인 유전자의 돌연변이가 확인되어야 한다. 하지만 우선적으로 후천 질환으로 인한 진행성 하지 강직을 배제하는 것이 무엇보다 중요하다. 감별이 필요한 주요 질환을
유전강직하반신마비 환자의 전기생리학적 검사에 대한 내용은 2016년 발표된 이탈리아 코호트 연구 결과를 소개하겠다[
대부분의 유전강직하반신마비 환자는 척수 MRI에서 특별한 이상 소견을 보이지 않고, 척수의 경미한 위축 정도만 관찰된다[
보고에 따라 차이가 있으나 유전강직하반신마비 환자의 13-40%에서 가족력이 없는 것으로 알려져 있다[
전체 유전강직하반신마비 중 가장 흔한 형태로, SPAST 유전자 변이에 의해 발생한다[
두 번째로 흔한 상염색체 우성 유전강직하반신마비로, Atlastin 단백질을 암호화하는 ATL1 유전자 변이에 의해 발생한다. Atlastin은 소포체(endoplasmic reticulum) 막에 존재하는 GTPase로 소포체 막의 homotypic fusion을 매개하는 역할을 한다. SPG3A의 임상 양상은 SPG4와 거의 유사하나 축삭형 말초신경병이 조금 더 흔하게 보고된다(25%) [
세 번째로 흔한 상염색체 우성 유전강직하반신마비로, 미토콘드리아 단백질인 receptor expression–enhancing protein 1을 암호화하는 REEP1 유전자 변이에 의해 발생한다[
미세소관에 결합하여 소포(vesicle) 및 세포소기관을 운반하는 역할을 하는 Kinesin-like protein KIF1A 단백질을 암호화하는 KIF1A 유전자 변이에 의해 발생한다. 상염색체 우성 유전으로 분류되었으나, 드물게 상염색체 열성으로 유전되기도 한다[
상염색체 열성 유전강직하반신마비 중 가장 흔한 형태로, Spatacsin 단백질을 암호화하는 SPG11 유전자 변이에 의해 발생한다. 발병 연령은 4-36세로 보고되고 있으며 임상 양상이 매우 다양하다. 대부분의 환자에서 인지기능저하가 동반되고 심지어 하지 강직보다 먼저 나타나는 것으로 알려져 있다[
SPG5는 cytochrome P-450 oxysterol 7a-hydroxylase를 암호화하는 CYP7B1 유전자 변이에 의해 발생한다[
Paraplegin 단백질을 암호화하는 SPG7 유전자 변이에 의해 발생한다. 발병 연령이 평균 41.7세로 늦은 편이며 여자보다 남자가 조금 더 많다[
Spastizin 단백질을 암호화하는 ZFYVE26 유전자 변이에 의해 발생한다. 임상 양상은 SPG11과 유사하나 축삭형 말초신경병과 파킨슨 증상이 조금 더 흔하게 동반된다[
드물게 전체 유전강직하반신마비 환자의 1-2% 미만에서 X염색체 연관 또는 모계 유전 방식을 따르는 유전자 변이가 보고된다.
이상으로 유전강직하반신마비의 임상 양상과 유전자 검사를 통한 진단 과정에 대해 알아보았다. 유전강직하반신마비의 진단을 위해서는 치료 방법이 있는 후천 질환과 일부 유전 질환을 감별하는 것이 중요하다. 최근 NGS 유전자 패널 검사가 점차 활성화되고 있으나, 가장 흔한 SPG4 원인 유전자인 SPAST 엑손 결실과 같은 변이는 NGS 검사에서 발견하기 어렵고, 그 외 뚜렷한 MRI 이상이 동반되지 않는 다른 유전 질환 또한 감별해야 하기 때문에 유전강직하반신마비의 유전형-표현형 상관관계를 정확히 이해하는 것이 중요하다.
